ED and Heart Health: Why the Treatment Conversation Should Go Further
Improving erections can be the immediate goal, while a broader assessment helps identify health issues worth addressing.

An effective treatment for erectile dysfunction can make an important difference in intimacy and confidence. Sildenafil and tadalafil, both listed by Haptic, are established options for appropriate patients. A good evaluation also asks why erections changed.
That question does not turn every episode of ED into a diagnosis of heart disease. It recognizes that sexual health belongs within a person's overall medical care.
Why clinicians consider cardiovascular health
The Princeton IV consensus brings together sexual medicine and cardiovascular experts, including Robert Kloner and colleagues. It treats ED as a potential marker of cardiovascular risk and recommends assessing the broader risk picture. Selected patients may benefit from additional evaluation, but no single heart test is required for everyone with ED. Princeton IV consensus
A useful assessment may consider blood pressure, diabetes, cholesterol, smoking, exercise tolerance, and family history. The appropriate next step depends on the complete picture, including whether symptoms suggest that sexual activity itself needs further assessment.
The benefit of this conversation is practical: a treatable risk factor may otherwise remain unaddressed while attention stays entirely on the prescription.
The cause may involve more than circulation
NIDDK describes neurological, hormonal, medication-related, psychological, and lifestyle contributors to ED. A new problem after starting a medication, for example, leads to a different discussion from a gradual change alongside diabetes. Stress and relationship factors can also matter. NIDDK causes of ED
Describe when the problem occurs, whether it is consistent, and what else changed around the same time. The aim is a useful history, not a test of masculinity or motivation.
What sildenafil and tadalafil can do
PDE5 inhibitors support the blood-flow response involved in an erection. They still require sexual stimulation. Choosing between options can involve duration of effect, timing, daily versus as-needed treatment, other medicines, and tolerability. NIDDK treatment overview
If the first experience is disappointing, describe exactly how the medication was used and what happened. The clinician can review the treatment rather than having the patient combine products or increase a dose independently.
Put interactions on the table
Nitrates and PDE5 inhibitors can cause dangerous blood-pressure lowering when combined. This includes nitrate medicines and recreational nitrites, often called poppers. Riociguat is another contraindicated combination. Blood-pressure medicines and alpha blockers also require review. Tadalafil prescribing information
If chest pain occurs after taking an ED medication, seek emergency care and tell responders which drug was taken and when. An erection lasting more than four hours also needs emergency assessment. Sudden hearing or vision loss requires prompt medical attention.

Mohit Khera's discussion with Peter Attia is one example of a substantive sexual-health conversation. It can make a medical appointment less intimidating and introduce questions worth asking. It does not establish an endorsement of Haptic, and it cannot account for an individual patient's contraindications. Original expert conversation
Research outlook: what may come next
Some observational research has associated PDE5-inhibitor use with better cardiovascular outcomes. Association does not establish that the drugs prevent heart attacks or prolong life. Future randomized evidence would be needed to support that kind of prescribing claim. Current use should remain tied to an appropriate indication and an individualized assessment.
